What Is the Discovery Nurture@Home Benefit?
The Nurture@Home benefit is a Discovery Health benefit designed to support new parents after returning home from hospital.
Eligible members receive access to professional nurse-led support focused on:
Postnatal recovery
Newborn feeding and routines
Emotional adjustment after birth
Early identification of concerns
The benefit includes three structured sessions, delivered at home and virtually, to support a healthy and confident transition into early parenthood.

What Happens During Your CareChamp Nurture@Home Visits
Your three sessions with CareChamp form part of a structured postnatal wellness screening, designed to assess wellbeing, identify early concerns, and provide preventative guidance for both mom and baby.
Face-to-Face Home Visit: Comprehensive Wellness Screening
During your in-person home visit, the nurse will:
Assess mom’s physical recovery after birth
Screen for postnatal emotional wellbeing and adjustment
Assess baby’s general health, feeding, behaviour, and settling
Observe feeding and routines to identify early challenges
Provide guidance on newborn care and safe practices
Identify any concerns that may need follow-up or referral
This visit allows for hands-on observation and early identification of potential issues.
Two Virtual Follow-Up Visits: Ongoing Wellness Monitoring
The two virtual consultations provide continued support and monitoring in the weeks following your home visit.
During these sessions, the nurse will:
Monitor feeding, sleep, and routine progress
Check in on mom’s emotional and physical wellbeing
Review baby’s development and adjustment at home
Address new concerns or questions
Provide guidance as your baby grows and your needs change
These sessions ensure continuity of care and ongoing professional oversight.
